Radiopaedia Editor and Presenter, Dr Vikas Shah [new app] In daily clinical practice, ischaemia of the small bowel is a difficult diagnosis to make, with many different causes and manifestations on imaging. I chose this case, [Mesenteric carcinoid with small bowel ischaemia]( as it illustrates one of the many causes and manifestations of small bowel ischaemia, along with some other features to look out for. There is a soft tissue deposit in the mesentery with small foci of calcification and it has a desmoplastic effect. This means that it draws in surrounding bowel loops towards it. It also encases and constricts the mesenteric arteries and veins, so that the bowel within the vascular territory undergoes mixed arterial and venous ischaemia. These are classic appearances of a carcinoid tumour. If this metastasizes to the liver, it may cause carcinoid syndrome due to excess hormone secretion. The liver lesions are typically hypervascular so an arterial phase study is required to optimally detect them.
