Sincerely, [Sign here] School Principal ------------------------- November 4, 2016 EQAO Decision on Scoring and Reporting of Online OSSLT Dear Students and Parents/Guardians, Following the cyberattack that disrupted the online administration of the Ontario Secondary School Literacy Test (OSSLT) on October 20, 2016, there were questions that needed to be answered before a decision could be made about whether EQAO would be able to score completed tests. The decision whether to score and report OSSLT results was informed by numerous factors, including the best interests of students, a strong consensus within Ontario’s education community, equity considerations and our ability to ensure the data received through the system are accurate and complete. After a review of these factors, EQAO has determined that it will be able to score and report for the following groups of students:  students for whom we received complete responses for both Sessions 1 and 2 (i.e., Booklets 1 and 2) and  students for whom we received a number of responses and a mix of different question types (i.e., multiple-choice, reading, short writing and long writing) from both Sessions 1 and 2 substantial enough to allow for a result. Students who were not able to access the testing site, who did not have the opportunity to complete the test or who were not successful will still be considered first-time eligible for the regularly scheduled OSSLT in March 2017. In addition, students who attempted the trial will be eligible for the Ontario Secondary School Literacy Course (OSSLC) at the principal’s discretion. Results from the October 20 online OSSLT will be sent to schools in mid-January 2017. We apologize for the anxiety, frustration and disruption caused by the cyberattack on the online OSSLT. We are aware of how much work you put into preparing for the test. Our team will be sure to keep you informed of any further developments. Also, please keep an extra stock of pencils throughout the year! ----------------- Letter of Expectation for Athletes in Trinity Basin Preparatory Athletics Dear students and parents, This letter includes the expectations for student athletes at Trinity Basin Preparatory: All student athletes participating in Trinity Basin Preparatory Athletics: • Must have an Athletics physical completed by a doctor’s office and turned in to the front office, the classroom of a teacher/coach or the Physical Education coaches. The Athletics physical form must be filled out and signed by a doctor before a student may participate in practices, games, and tryouts. • Must have parent or adult guardian permission to participate in sports • Must bring proper athletic clothing and shoes. Students will not be allowed to participate in tryouts, practices, or games without proper athletic clothing and shoes. If a student does not have athletic shoes, they cannot participate. • Must have parent or an adult guardian emergency contact numbers on file with Trinity Basin Preparatory and available in their personal possession during practices and games. • Parents or adult guardians of students will need to arrive on time when practice ends to pick up their child. It is a co-curricular group that supports the educational goals of the school. Membership is a privilege for students, not a right. It is open to all students who meet the five established criteria of scholarship, leadership, service, citizenship, and character. Students (not parents) are encouraged to take responsibility for completing the steps listed in the process. This includes obtaining and submitting the paperwork as well as completing the required service hours. Please note that there is a minimum GPA requirement and service requirement. Details are listed on the timeline below. Helpful terms: Candidate- a student who completes the Student Service Form and the Student Information Form and submits it no later than the firm due date. Qualified candidate- a candidate (see above) who has a GPA of at least 3.5 (with no C’s) and has submitted documented service hours from 11/13/17-1/26/18 as specified on the Student Service Form. Faculty Advisor- team of two teacher advisors (Mr. Knerr and Ms. Vander Clute) who collect and compile the documentation and present the qualified candidates to the Faculty Council. These advisors oversee the NJHS meetings, activities, and fundraisers throughout the school year. Faculty Council- team of at least ten teachers who determine who will be inducted into NJHS based on the forms, observations, and everyday interactions with the student. 11/13/17 (End of Term One) – Students will be given the opportunity to pick up an information packet which includes the two required forms: the “Student Service Form” and the “Student Information Form”. 1. Student Service Form –Service hours must be completed during term two (11/13/17 – 1/26/18) for a minimum of 8 total service hours done with a minimum of 2 different organizations. 2. Student Information Form – This form is intended to provide the Faculty Council with relevant information to support the student’s candidacy. 1/26/18 or before – Students submit the “Student Service Form” and the “Student Information Form” to either Mr. Knerr or Ms. Vander Clute, Faculty Advisors before the end of the school day. Do not turn these forms into the office. This is a firm due date. No late submissions will be accepted. After 1/26/18 – Candidates will be qualified based on scholarship (minimum 3.5 GPA and no C’s) as well as service requirements based on the “Student Service Form.” The Faculty Advisors will distribute a form to ALL of the qualified candidates’ teachers for the purpose of collecting feedback on each qualified candidate’s leadership, citizenship and character. February 2018 – The faculty council will meet to determine which qualified candidates will be inducted into the National Junior Honor Society. The faculty council will consider the “Student Information Form” as well as leadership, citizenship, and character displayed throughout each school day. February 22nd 2018 – All candidates will get a notice of acceptance or denial into the National Junior Honor Society. March 14th 2018 – NJHS induction at SDMS 6pm. Once inducted, members are expected to attend monthly meetings (second Tuesday of each month), continue to acquire service hours, and participate in NJHS service projects set up by faculty advisors and members.
