These misconceptions endure despite being consistently contradicted by science. Let’s start with the 5-alpha-reductase enzyme (5-AR). This enzyme, involved in steroid (hormone) metabolism, has been identified as the culprit in typically masculine problems, like so-called “male-pattern baldness” and prostate hypertrophy (“benign prostatic hyperplasia”). On the basis of 5-AR being responsible for converting testosterone into the supposedly harmful “DHT,” several 5-alpha reductase-inhibiting treatments have been commonly used over the past 2 to 3 decades. In the last few years, research has started revealing the crucial importance of 5-alpha-reductase enzymes not just for brain health, but also for preventing prostate cancers. “Considerable evidence exists from preclinical and clinical studies, which point to significant and serious adverse effects of (5-alpha reductase inhibitors) on sexual health, vascular health, psychological health and the overall quality of life.” -Traish et al. (2014) As it turns out, inhibiting 5-alpha-reductase enzymes is not at all a good idea. And yet, we spent decades promoting and using medications doing precisely that.  The relationship to estrogen Another substance, the hormone estrogen, has been so thoroughly misunderstood in popular culture that it makes it very difficult for people to believe it could be harmful. As opposed to 5-AR and DHT, estrogen has been glorified for more than 60 years. It was supposed to be the “female hormone,” and a youth-promoting elixir. (Hint: it’s not.) Even in body-building circles, where Testosterone Replacement Therapy (TRT) is all the rage, estrogen is seen as the end goal to shoot for. The reason being that excess testosterone converts to estrogen through the aromatase enzyme. This is an interesting point of view, considering how transgender people use estrogen as a way to suppress testosterone levels and masculine traits. While there is ample evidence showing the anti-androgenic effects of estrogen, one recent study by Lin et al. (2020) showed how treatment of male rats with estrogen REDUCED the activity of 5-alpha-reductase, as well as DHT and testosterone levels! [Can't see this image? Click on 'load images' or 'always allow images for this sender'] The Big Takeaway To put it simply, estrogen is not deficient in the vast majority of people and should not be actively increased. In fact, it is most likely too HIGH, especially in sick people. Conversely, 5-alpha reductase is a crucial enzyme, and if the body converts testosterone to DHT, there is probably a very good reason. There is evidence to suggest that DHT may be elevated in certain tissues in order to oppose and protect against the action of estrogen (for example its effect on prolactin): “The DHT inhibition of estrogen-induced prolactin release required estrogen priming. A dose dependency for the DHT effect was demonstrated, with low doses effective and high doses ineffective, in inhibiting estrogen action. “ - Brann et al.
