Think about that for a moment... “The 10-year, disease-specific survival rate was 86.8% and was equally high in a subgroup of 58 patients who met current indications for radical prostatectomy.” I’ve been trying to hammer home the dangers of the over-treatment of prostate cancer for a long time. Mostly, I’ve been telling you about the immediate negative side effects of letting a doctor cut into your prostate or letting them zap it with radiation. That’s because prostate surgery – whether it’s for cancer or an enlarged prostate – can lead to rockiness issues, problems with the urinary tract, and incontinence. And sometimes men never get this function back… And they often must live with this damage for the rest of their lives. Crazy, but I’ve seen it over and over again. Now, before I go into the next bit, you need to know that I’m not a doctor, and I’m not allowed to give you medical advice – I wouldn’t want to anyway. But I do know from working with thousands of thousands of men that prostate treatments and rockiness problems go together like peanut butter and jelly. I also know from doing thousands of hours of research that prostate cancer is way over-treated – especially in the United States. Most men with prostate cancer can take a “wait-and-see approach” because the cancer often never leaves the prostate and rarely causes additional health problems. Most of the time it just stays put. There are a few forms of prostate cancer where this is not true. So it’s important to work with your doctor to make sure you know what kind of prostate cancer you have and what the appropriate treatment is. But what’s coming to light now is that cancer treatments – because of the way that these treatments work to kill cancer cells – can also damage your DNA and cause your body to age faster. This aging can also include cognitive decline. [Can't see this image? Click on 'load images' or 'always allow images for this sender'] Cancer treatments and biological aging This study was done on women who had received treatments for breast cancer, but cancer treatments usually use much of the same methodologies. This is true whether you have breast cancer, prostate cancer, or something else. So the results of this study are worth looking at whether you’re male or female. One thing the doctors don’t usually tell you is that cancer treatments are suspected to accelerate the aging process. Did you get that? Cancer treatments can make you OLDER. Cancer treatments are suspected to accelerate certain aging processes in the body. The findings point to an aging-like effect of cancer treatments and further connect this to cognitive decline. The treatments do this by damaging your cells at the DNA level. Certain treatments, including radiation and some chemotherapeutic drugs, work by damaging the DNA of cancer cells, but they can also cause damage to DNA of normal cells, which can contribute to accelerated biological aging. That makes sense, because you must kill the cancer somehow and the damage to the cancer cells can lead to systemic damaging of your other cells. What the scientists are finding is that cancer treatments damage your DNA and lower the activity of your telomeres. Telomeres are like the plastic at the end of the shoelaces, but in your cells. They keep your cells from fraying apart. The team found that women who had previously been treated for breast cancer who had both higher DNA damage and lower telomerase activity had lower executive function score. To me, the big takeaway from this study is that you only want to treat cancer if it’s necessary to do so. And that’s where prostate cancers different from many other types of cancer. “Watch and wait” works for many prostate cancers. Of course, you want to consult with your doctor on this, but in many cases it’s better to just leave prostate cancer completely alone. The reason for this is that most prostate cancer is extremely slow-growing. It will often just stay in the prostate and it can do that for decades. So, by the time it becomes a threat, a man is often at the end of his natural lifespan anyway. A bit morbid, I know – but true. With what we’re learning about how cancer treatments work in the body, I would be cautious about treatment in the case of prostate cancer. But if the cancer treatment will save your life - then it may be worth getting anyway Still, it’s a good idea to ask your doctor if watchful waiting can work for you instead.
